Overdose

Symptoms: dizziness, motor restlessness, impaired consciousness.

Treatment: to give the patient a horizontal position, intravenous administration of barbiturates (with muscle twitching or generalized convulsions), intravenous infusion of balanced electrolyte and plasma-substituting solutions, intravenous administration of glucocorticoids, epinephrine (in case of threatening vascular collapse and increasing bradycardia), oxygen supply, artificial respiration, if necessary, endotracheal intubation with controlled ventilation of the lungs, the use of peripheral vasodilators with increased blood pressure.

Contraindications

Hypersensitivity, paroxysmal tachycardia, tachyarrhythmia, bronchial asthma, angle-closure glaucoma, simultaneous administration of non-selective beta-blockers, severe hepatic insufficiency, porphyria, intravascular injections, hyperthyroidism, simultaneous administration of MAO inhibitors or tricyclic antidepressants, CNS diseases, malignant anemia.

Incompatibilities

The hypertensive effect can be enhanced by tricyclic antidepressants and MAO inhibitors. Do not prescribe the drug against the background of treatment with non-selective beta-blockers due to the high risk of developing a hypertensive crisis and severe bradycardia.

Undesirable effects

From the nervous system and sensory organs: headache, possible cases of impaired consciousness, up to its loss, respiratory disorders, up to its stop, twitching and tremor of the muscles, convulsions, rarely — blurred vision, diplopia, transient blindness.

From the cardiovascular system and blood (hematopoiesis, hemostasis): rarely-arterial hypotension, tachycardia, bradycardia, depression of cardiovascular activity.

From the respiratory system: dyspnea, apnea.

From the gastrointestinal tract: nausea, vomiting.

Allergic reactions: rhinitis, skin hyperemia, itching, angioedema, edema and inflammation at the injection site, rarely-anaphylactic shock.

Special instructions

When removing the premolars of the lower jaw in the uncomplicated stage, infiltration anesthesia gives the effect of conducting anesthesia.

Pharmacokinetic properties

Articaine binds to plasma proteins by 95%, is metabolized in the liver, is excreted by the kidneys (54-63% in 6 hours), T1/225 min. Penetrates through the placental barrier.

Dosage (Posology) and method of administration

For uncomplicated removal of the upper jaw teeth-1.8 ml (vestibular depot) per tooth, if necessary — an additional 1-1. 8 ml, for a palatine incision or suture-0.1 ml( palatine depot), when preparing cavities and turning teeth under crowns (except for the molars of the lower jaw) - 0.5-1.8 ml per tooth (vestibular injection). When performing one treatment procedure, adults can be administered up to 7 mg of articaine per 1 kg of body weight. The duration of anesthesia, during which the intervention can be performed, is 30-45 minutes.
