Contraindications

Absolute values: hypersensitivity, shock, bed rest, diseases caused by a blockage of the artery of the III-IV degree, chronic arterial insufficiency, acute inflammatory diseases in the area of sclerosis.

Relative values: thrombosis of superficial and deep (especially) veins, edema, symptoms of diabetic microangiopathy, acute and chronic heart diseases (endocarditis, myocarditis), febrile conditions, diseases caused by a blockage of the artery of the second degree (with cosmetic indications), bronchial asthma.

Incompatibilities

Increases the arrhythmogenic effect of general anesthetics.

Undesirable effects

When sclerotherapy of varicose veins of the extremities: hyperpigmentation of the skin at the injection site.

With sclerosis of hemorrhoids: transient pains.

With sclerosis of varicose veins of the esophagus and stomach: necrosis, ulceration and stenosis of the esophagus, exudation into the pleural cavity, dysphagia, fever, anaphylactic shock.

Special instructions

When sclerosing subcutaneous veins and telangiectasias, the injection site is pressed with a tampon and a compression bandage is immediately applied, after the procedure, the patient should walk for 30 minutes. The bandage compresses the walls of the veins, which prevents recanalization and promotes the formation of a fibrous scar. When sclerosing telangiectasias, the bandage is applied for 2-3 days, with severe varicose veins, compression with elastic bandages or medical stockings of classes II-III is recommended for 4-6 weeks.

Nature and contents of container

1 ml of 0.5% solution for injection contains polydocanol 5 mg, 1 ml of 1% solution-10 mg, in ampoules of 2 ml or vials of 30 ml, in a box of 5 ampoules or 1 bottle.

1 ml of 2% solution contains polydocanol 20 mg, 3% solution-30 mg, in ampoules of 2 ml, in a box of 5 ampoules.

Fertility, pregnancy and lactation

Contraindicated in the first trimester of pregnancy and after 36 weeks, breastfeeding after administration of the drug is recommended to stop for 2-3 days.

Dosage (Posology) and method of administration

The total dose of polydocanol for sclerosis of varicose veins and hemorrhoids should not exceed 2 mg/kg of body weight per day. For a patient weighing 70 kg, up to 140 mg of polydocanol can be administered (28 ml of 0.5% ethoxysclerol solution, 14 ml of 1% ethoxysclerol solution, 4.6 ml of 3% ethoxysclerol solution).

Sclerosis of varicose veins and telangiectasias: strictly in / in with a horizontal or raised by 30-45 ° leg position, apply 0.5-3% solution in ampoules.

Sclerosis of subcutaneous varicose veins: 0.1-0.3 ml of 0.5% solution per injection (depending on the size of the sclerosed veins).

Sclerotherapy of telangiectasias: 0.1–0.2 ml of 0.5% solution in a single injection.

Sclerosis of the central vein in telangiectasis: 0.1-0.2 ml of 1% solution per injection.

Sclerosis of small varicose veins: 0.1-0.3 ml of 1% solution per injection.

Sclerosis of medium-sized varicose veins: 0.5-1 ml of 2-3% solution per injection. At the first reception (especially in patients prone to hypersensitivity reactions), one injection is made, at the next one it can be administered several times, taking into account the maximum dose.

Sclerosis of hemorrhoids: use a 3% solution in ampoules, the first single dose-0.5-2 ml of 3% solution strictly under the mucous membrane above the node (in men, with a node located at 11 h, no more than 0.5 ml is administered), again - after 1-2 weeks, 3 ml of 3% solution.

Sclerosis of varicose veins of the esophagus and stomach. The total dose of polydocanol should not exceed 4 mg/kg of body weight. Use 0.5-1% solution in vials, mainly paravasally (submucosally, subepithelially) in the lower third of the esophagus (intravasal and combined — para - and intravasal injections are possible). With paravasal administration, it causes local edema, squeezing varicose veins and facilitating scarring of the esophageal wall. In case of bleeding — 5-15 ml (up to 30 ml) of 0.5% solution in 1 ml portions until the bleeding stops, in the absence of bleeding-paravasally up to 40 ml of 0.5% solution in 1 ml portions or intravasally-no more than 20 ml in 2 ml portions, with an interval of 5-7 days, the procedure is repeated 2-4 times using 1% solution
